In Part 1 of this series, we explored the powerful link between testosterone and your libido. Once you and your doctor have confirmed that Low Testosterone is affecting you and decided that Testosterone Replacement Therapy (TRT) is the right path, the next question is practical: how is it administered?

This guide will walk you through the most common methods including injections, gels, and pills, so you can understand how each works and have an informed discussion with your doctor.

The Importance of Medical Supervision

Before we detail the methods, one point is critical: TRT is a medical treatment that must be prescribed and monitored by a qualified doctor. Self-prescribing or obtaining testosterone from unregulated sources is dangerous. Your dosage, method, and progress must be professionally managed to ensure safety and effectiveness.

Method 1: Testosterone Injections

Testosterone injections are a common, effective, and often cost-effective method for TRT. They involve injecting testosterone directly into a muscle (intramuscular) or into the fat layer under the skin (subcutaneous).

How to Self-Administer Injections Safely: Your doctor will provide full training, but here is a general overview of the steps:

  1. Gather Supplies: You'll need your vial of testosterone, a sterile syringe and needle, and alcohol swabs.

  2. Prepare the Dose: Clean the top of the vial with an alcohol swab. Draw the prescribed amount of testosterone into the syringe.

  3. Choose and Clean the Site: Common injection sites include the gluteal muscle (upper buttock) or the thigh muscle. Clean the skin thoroughly with a new alcohol swab.

  4. Perform the Injection: Insert the needle at a 90-degree angle into the muscle and slowly push the plunger to inject the medication.

  5. Dispose Safely: Never reuse a needle. Dispose of the syringe and needle immediately in a designated sharps container.

Injections are typically done on a weekly or bi-weekly schedule.

Method 2: Topical Testosterone Gels

Gels are a popular needle-free option. The testosterone is absorbed daily through the skin and into the bloodstream.

How to Apply Gels Correctly:

  1. Start with Clean, Dry Skin: Apply the gel after a shower once your skin is completely dry.

  2. Apply the Dose: Use the pump or packet to dispense the prescribed amount of gel onto your hand.

  3. Rub it in: Apply a thin layer to your shoulders, upper arms, and/or abdomen. Do not apply it to your genitals.

  4. Let it Dry: Allow the gel to dry completely for several minutes before dressing.

  5. Wash Your Hands: Thoroughly wash your hands with soap and water after application.

Crucial Safety Warning: The primary risk with gels is transference. You must avoid skin-to-skin contact with women and children on the application area, as the testosterone can be absorbed into their bodies. Keep the area covered with clothing.

Method 3: Testosterone Pills (Capsules)

Oral testosterone capsules are a convenient, needle-free option. Modern formulations are designed to be absorbed through the lymphatic system, which helps avoid the liver strain associated with older types of testosterone pills.

How to Take Testosterone Capsules:

  • Follow the Prescription: Capsules are typically taken twice per day, once in the morning and once in the evening.

  • Take with Food: They must be taken with a meal that contains some fat to ensure proper absorption.

  • Consistency is Key: Take your capsules at the same times each day to maintain stable hormone levels.

Which Method is Right for You?

The best method depends on your lifestyle, preference, and your doctor's recommendation.

  • Injections:

    • Pros: Highly effective, cost-effective, less frequent dosing.

    • Cons: Requires comfort with needles.

  • Gels:

    • Pros: Painless, provides stable daily hormone levels.

    • Cons: Requires daily application, risk of transference to others.

  • Pills:

    • Pros: Very convenient, no needles or drying time.

    • Cons: Must be taken with food, usually twice a day.
